Who is Cipher supposed to be in MGSV

I don't understand who exactly is referred to as Cipher in this game. Is it: 1. Skullface and xof 2. Zero ( but isn't he already in vegetative state by this game and if he isn't, why would he even pursue Diamond Dogs) 3. AI system prepared by Zero
